| Firearms Act - Application to be deemed a non-prohibited person | Application form to be completed in order to make an application to be permitted to be the holder of a firearm licence. This form can be used if deemed to be a prohibited person as a result of the making of an Intervention Order or as a result of a criminal determination. |

| FVIO4 Application to Extend, Vary or Revoke an Intervention Order | This form is to be completed if the applicant or respondent are seeking to change, cancel or extend the current intervention order. |
| FVIO8 Application for Rehearing | This form is to be completed if the respondent is seeking a rehearing of the application for an intervention order. |
| FVIO9 Notice of Withdrawal of an Intervention Order | This form is to be completed and filed with the court if the applicant wishes to withdraw their application. |
| Information form for a Personal Safety Intervention Order (PSI01) | This form is to be completed to make an application for a personal safety intervention order. |
